Separate Negotiations by Provider, and Insurance Type.

Currently inside of Neos, whenever there are multiple entries on the Negotiation from the same provider regardless of insurance type, all of the entries will be grouped together. It is very often that the same insurance provider will have multiple types of insurances (IE: Pip, UIM, etc).

Without this change, when there is the same insurance provider that provides different insurances will be put into one group making it tedious, and time-consuming to find specific negotiations for specific insurance types.
