Intake name blocks no longer contain prefix/suffix fields

After the intake update, the name block no longer has prefix/suffix fields listed. The name block cannot be modified, so the previous settings cannot be restored. This will help improve visibility with having the prefix/suffix as a part of the name field, not a separate field below/above.
