Make More of Provider Name Visible For Letter Templates

Allow us to scroll right to see the full provider name when we are on the template screen to make letters. This will allow us to be sure we are selecting the correct provider when we create letters as clients often go to several providers and the beginning of the provider names often only tells us if it's a bill or medical record entry. Currently, we have to guess which provider is which and create the letter and this causes us to create multiple incorrect letters before finally selecting the correct provider. It is a large waste of time.
