Needles 5 Word Addin Additions

Currently inside of the Needles 5 MS Word add-in, you are able to post the file to the case; however, there is no way for a user to be able to choose a category or any additional information for this document to come over with.

It would be best to be able to fill out all of the information that is available inside of the application window before we are able to post to the case.
