Add clinic/office for provider and display it when searching

With providers, it would be helpful to have a field indicating what clinic a provider is working for and it would be especially helpful if that information appears alongside info like “address” and “case” when you search or at least displayed in the small window/tile when you click a search result once, that would speed things up.
