want user defined fields to add yes/no or checkmark items for things such as

1. body worn camera footage

2. scene photographs taken?

3. witness statements done?

4. diagrams?

5. third party investigation done? (OSHA, taxicab authority, etc)

then also having the ability to docgen a request for said documents/items.
