Auto name Medical Record Requests based on Provider / Party

Before the last Neos update, Documents with multiple Parties / Providers would generate with with dynamic names based on the Party / Provider.

With the new Document Generation system, you need to manually change the name of each unique copy of the document.

Please allow documents to be automatically named based on provider / party / template name
